Cetonemia por fome prolongada e síndrome confusional

Abstract

O “caso social” em saúde é descrito como a pessoa numa dada situação e o seu estado de saúde, numa visão holística e integrando as condições pessoais, sociais e ambientais. As autoras descrevem o caso social de uma idosa de mais de 80 anos, encaminhada por um Mandado de Condução da PSP ao Serviço de Urgência na sequência da sinalização dos vizinhos. Este facto motivou o internamento com diagnóstico de cetonemia por fome prolongada e síndrome confusional (uma condição clínica resultante de desnutrição que motivou a deterioração do estado de consciência) e para resolução social. Existia já anteriormente suporte da rede formal, mas sem permissão da própria para intervenção. Através de um trabalho interdisciplinar e em rede comunitária e o envolvimento da família, foi possível durante o internamento encontrar uma resposta social adequada às necessidades da doente através da integração em Estrutura Residencial para Idosos (ERPI), assegurando o seu bem-estar físico e mental. Neste sentido, foi fundamental a sinalização da situação ao Ministério Público no âmbito do Regime de Maior Acompanhado. Com este artigo pretende-se a descrição e reflexão sobre o caso social e as suas dimensões, bem como enquadrar e relacionar com as políticas e respostas sociais e jurídicas no exercício do assistente social integrado numa equipa multidisciplinar hospitalar.

The “social case” in health is described as the person in each situation and their state of health, in a holistic view and integrating personal, social, and environmental conditions. The authors describe the social case of an 81-year-old woman, referred by a PSP Driving Warrant to the Emergency Service following the neighbors’ signaling. This fact led to hospitalization with a diagnosis of ketonemia due to prolonged hunger and confusional syndrome (a clinical condition resulting from malnutrition that led to the deterioration of the state of consciousness) and for social resolution. With prior formal support from the network, but without permission from the network itself for intervention. Through interdisciplinary work and community networking and family involvement, it was possible to find an adequate social response to needs with integration into Residential Structure for the Elderly (ERPI), ensuring their physical and mental well-being. In this sense, it was fundamental to signal the situation to the Public Prosecutor’s Office within the scope of the Regime for Greater Accompanied. The aim of this article is to describe and reflect on the social case and its dimensions, as well as frame and relate it to social and legal policies and responses in the practice of social workers as part of a multidisciplinary hospital team.
