Título: A musicoterapia como promotora do alívio da dor crónica

Resumo: Ao evoluir para uma situação crónica, a dor torna-se um problema de saúde na medida em que é prolongada no tempo, de causa desconhecida e afeta o indivíduo nos seus domínios físico, cognitivo, psicológico e comportamental. Este tipo de dor é responsável por um elevado nível de morbilidade, absentismo e incapacidade temporária ou permanente, gerando elevados custos aos sistemas de saúde. Por estes motivos, e por condicionar bastante a qualidade de vida da pessoa, a dor crónica deve ser valorizada e tratada com farmacoterapia e com as diferentes terapias existentes que têm demonstrado sucesso no seu alívio. A musicoterapia, atuando nas necessidades físicas e psicossociais do ser humano, surge como uma terapia com resultados científicos comprovados no alívio da dor crónica. Através da utilização das suas diferentes técnicas e dos seus efeitos comunicativo, distrativo e de autoexpressão, aliados aos efeitos neurofisiológicos que a música induz no SNC, permite a mudança de foco, aparentando ser uma explicação plausível para o seu efeito analgésico. Além disso, auxilia na execução das tarefas, aumenta a perceção e autoestima, conecta pensamentos e sentimentos, e favorece as relações interpessoais. Neste sentido, e no âmbito do estágio curricular do mestrado em musicoterapia decorrido entre outubro de 2018 e junho de 2019, foram realizadas intervenções musicoterapêuticas com 20 utentes que frequentam a Unidade de Tratamento da Dor do Hospital Lusíadas de Lisboa. Neste documento serão descritas as intervenções musicoterapêuticas realizadas no âmbito do referido estágio e os dados do progresso terapêutico dos utentes recolhidos através de dados de observação clínica e instrumentos de avaliação quantitativos. Para comprovar os resultados benéficos da musicoterapia adquiridos ao longo de várias sessões, foram abordados dois dos casos intervencionados em formato de estudo de caso. Foi possível verificar que as intervenções musicoterapêuticas, particularmente o canto e escrita de letras para músicas da sua preferência, contribuíram para a diminuição e alteração do foco da dor, recuperação de memórias antigas e recentes, aumento da autoestima e melhoria das relações interpessoais apresentando, assim, melhorias nos domínios emocional, cognitivo-intelectual, adaptação social e processos cognitivos.
When developing into a chronic situation, pain becomes a health problem because it extends through time, its cause is unknown and it affects the individual in his/her physical, cognitive, psychological and behavioral domains. This type of pain is responsible for a high level of morbidity, absenteeism and temporary or permanent incapacity, causing high costs to health systems. For these and because it greatly affects the person’s quality of life , chronic pain should be valued and treated with pharmacotherapy and with the different types of therapies that have demonstrated success in their relief. Music therapy, by acting on the physical and psychosocial needs of the human being, emerges as a therapy with proven scientific results in the relief of chronic pain. Through the use of its different techniques and its communicative, distracting and self-expression effects, together with the neurophysiological effects that music induces in the CNS, it allows the change of focus, appearing to be a plausible reason for its analgesic effect. In addition, it helps in the execution of tasks, increases the perception and self-esteem, connects thoughts and feelings, and fosters interpersonal relations. In this sense, and within the scope of the curricular internship of the master's degree in music therapy that took place between October 2018 and June 2019, music therapy interventions were performed with 20 participants attending the unit for pain treatment at Hospital dos Lusíadas, in Lisbon (Portugal). This document will describe the music therapy interventions performed within the scope of this stage and the data of therapeutic progress of the users that was studied through the clinical observation and application of quantitative scales. In order to prove the beneficial results of music therapy acquired over several sessions, two of the cases studied were explored in a case study format. It was possible to verify that musical therapy interventions, specifically the singing and writing of lyrics for their favorite musics, contributed to the decrease and alteration of the focus of pain, the recovery of old and new memories, the increase of self-esteem and the improvement in interpersonal relations, thus presenting improvements in the emotional and cognitive-intellectual domains, social adaptation and cognitive processes.
